Program Description
The Utah Children’s Health Insurance Program, or CHIP, provides medical services to eligible children under the age of 19 who do not have health insurance and do not qualify for Medicaid. Services provided under the CHIP program include: sick visits; well child exams; prescriptions; dental; hospital care; emergency room visits; urgent care visits; eye exams, and related services. CHIP emphasizes preventive care and there are no co-payment requirements for any preventive services (e.g., well-child checkups and immunizations). There are some co-payments required for other services and quarterly premiums. All costs are dependent on the family’s income. CHIP is administered by the Utah Department of Health, and currently contracts with Select Health and Molina to provide health care services.
Program Requirements
In order to qualify for CHIP, children must be U.S. citizens or legal aliens, residents of Utah, under 19 years of age, and not covered by health insurance or eligible for Medicaid.
In order to qualify, you must have an annual household income (before taxes) that is below the following amounts:
